Volunteer Agreement

In consideration of being allowed to participate in AZA ESSENTIALS NFP volunteer activities and all related activities, including any activities incidental to such participation (collectively referred to as "Volunteer Activities"), I, the undersigned volunteer, or the parent/legal guardian of the volunteer if the volunteer is under 18 (hereafter referred to as "I," "me," or "my"), release and agree not to sue AZA ESSENTIALS NFP, its officers, directors, employees, subcontractors, sponsors, agents, and affiliates (collectively, the "Foundation") from all present and future claims that may arise from my participation in the Volunteer Activities, including but not limited to claims for property damage, personal injury, or wrongful death, however caused.

I understand and agree that the Foundation is not responsible for any injury or property damage that may result from my participation in the Volunteer Activities, even if caused by ordinary negligence or otherwise.

I acknowledge that participation in the Volunteer Activities involves certain risks, including but not limited to serious injury and death. I voluntarily choose to participate with full knowledge of these risks and agree to assume all such risks. I further agree to indemnify and hold harmless the Foundation from any claims arising from my participation.

I understand that this document is intended to be as broad and inclusive as permitted by law, and if any portion is deemed invalid, the remainder shall remain in full legal force and effect.

I acknowledge that the Foundation does not provide any insurance coverage for me, my family, heirs, or assigns. I certify that, to my knowledge, I am in good health and do not have any physical conditions that would prevent my participation in Volunteer Activities.

I understand that this is a legally binding contract that waives certain legal rights. By signing below, I confirm that I have read and understand this agreement and that I am signing voluntarily.

In consideration of being allowed to participate in AZA ESSENTIALS NFP Volunteer Activities, I, the undersigned Volunteer or Parent/Legal Guardian of the Volunteer (if under 18), grant AZA ESSENTIALS NFP and its subsidiaries, affiliates, agents, advertising/promotional agencies, and partners (collectively, “Authorized Parties”) the absolute and irrevocable right to use, publish, broadcast, and/or copyright my name, address, voice, photograph, likeness, caricature, and personal information in its current form or modified in any way.

This authorization applies to any and all advertising, promotional, or other materials derived from the Volunteer Activities, in any media now known or developed in the future, worldwide and in perpetuity, without additional compensation. I understand that all materials created shall be the sole property of the Authorized Parties, and I waive any rights to inspect or approve the final product.

I agree not to authorize the use of any print, negative, or other copy thereof by anyone other than the Authorized Parties.

I acknowledge that this document is legally binding, and if any portion is deemed invalid, the remainder shall remain in full legal force and effect.
